Business Data Report for Hotels (011) and Restaurants (012)
For owners/agents of Restaurants or Hotels, your Annual Business Data Report must be received by the Department prior to your renewal being processed. The Business Data Report can now be completed and submitted via our website. Note: there is no additional fee to complete this report online below.
Upon completing your online Business Data Report, if you have not already completed your renewal, you can do that through our e-licensing portal, OR the paper renewal form downloadable at https://azliquor.gov/renewals.cfm.
